After you have spoken with our representative and been given a set date/time, we ask that you please complete the Booking Confirmation Form under the form and scales and then return it to us by email(firstname.lastname@example.org) or fax. Your appointment is not officially booked or confirmed until the Booking Confirmation Form has been submitted.
Initial evaluations are generally hour and a half. Please complete the intake form and submit it to us atleast 48 hours before the visit. The intake form can also be submitted through the email or fax. Completing the intake form ahead of time allows clients to save time and resources by collecting necessary diagnostic information ahead of time.
For children the initial evaluations consist of interview together with the family and individually with the child or teen. To make a diagnosis, sometimes additional appointments are needed to clarify diagnosis by collecting diagnostic lab work, additional scales or neuropsychological testing.
Because of high demand and a very busy schedule, we will only schedule initial evaluation if half of the initial evaluation payment is made at the time of setting the appointment. It is important to note that there a holding charge of 50% of the initial visit on credit card of new patients to officially book their appointments. If the patient cancels 48 hours before the visit they will receive 100% refund of their holding charge money back. This policy in place to give maximum time and availability to our current patients.
